Beginning April 29, 1998

Maintain on paper or in a file a log of team meetings that contains the following elements:

• Team Name
• Date
• Time Began
• Time Ended
• Attendance List
• Purpose of the Meeting
• Minutes of the Meeting
• Topics Discussed
• Open Issues
• Questions Asked/Answered
• Individual Assignments

If two or more sub-teams meet at the same time, make separate log sheets. Log sheets are expected to be about 1 to 2 pages.
